Many private insurance companies do cover consultation visits. Laboratory tests may be covered under your private insurance plan while natural remedies are generally not covered under most insurance plans. Each person’s insurance plan may vary and thus we recommend checking your individual plan for details. Currently OHIP does not cover Naturopathic, Chiropractic, Massage Therapy visits. You will be given a receipt of your services to submit to your insurance provider.
A credit card is required on file and a pre-payment is required to reserve your appointments including new patient appointments.
Cancellations and Missed Appointments
An appointment not kept means loss of valuable time and a spot that someone else in need may require. A charge in the amount for 100% of your missed visit booked will be made unless 48 business hours notice is given of cancellation. The rescheduled or cancelled appointment must be received by speaking directly to an Revivelife™ Front Staff member.
Please keep all of your receipts for insurance and income tax purposes. There is a $25.00 minimum reissuing fee for copies.
Payment of Account
Payment of your bill is required on the day of service. For your convenience, we accept exact cash, Interac, MasterCard and Visa.
All letters are $50/page and require 5 business days to complete.
Photocopies up to 5 pages are included in each visit. Any additional photocopies required are invoiced at 10 cents/copy.
We offer home delivery services from our dispensary. Delivery charges are $12.00 for local areas and $20.00 and up for further distance areas. Credit card payment is required for all deliveries. For same day service, orders must be placed and prepared before 12:00 noon.
Any requests for Special Orders (Items we do not regularly stock) or requests to hold items for pick up must be pre-paid at the time of request. We encourage you to call to request a refill of any of your dispensary items prior to running out to allow us time to order your items. Any prescription items require a consultation prior to refills please allow 7-14 business days following your visit to receive your prescription. Note-BHRT hormone refills may take up to 3 weeks from your appointment to your receipt of hormones, thus please schedule your follow up visits at least 3 weeks before you run out of your prescription.
Dispensary Return Policy
Please note that we have a thirty day return policy for all unopened supplements. We cannot return any opened, custom, or specially formulated remedies i.e. custom botanicals (liquids) or homeopathic remedies for health regulations purposes. Thank you for your understanding.
