SAD Questionnaire: Score each question from 0-3 with 0 being never- very infrequently experience to 3 being experience it significantly

YOUR TOTAL SCORE

Your Evaluation of SAD & Winter Blues:

  • If your total score is 0-10 there is a low risk that you have either SAD or the winter blues.
  • If your total score is 11-22 there is a moderate risk that you may be experiencing SAD and a high risk that you have the winter blues. It is recommended that you follow up with your health care team for proper assessment and support.
  • If your total score is 23-33 there is a high risk that you may be experiencing SAD. It is recommended that you follow up with your health care team for proper assessment and support.
