Scheduling
Please note that a credit card on file is required to schedule an appointment and will be charged in the case of less than 48-hour notice of cancellation of an appointment. Appointments may be made by the following methods:

Cancellations
You will receive a email reminder 3 days before your appointment reminding you of your visit. We reserve scheduled appointments specially for you. Because our visits lasts much longer than traditional doctor visits, cancellations/no-shows cause significant scheduling interruption. Please note that we require a minimum if 48-hours (2 business days) notice for appointment changes. If we do not receive adequate notice of appointment changes (48 hours/ 2 business days), you will be charged the full amount of a missed, no-show or last minute cancellation. This allows live-time access to other patients who maybe waiting. We will always attempt to contact you first prior to collecting fees to discuss any extenuating circumstances leading to the missed appointment. Thank you for your understanding
Receipts
Please keep all of your receipts for insurance and income tax purposes. There is a $25.00 minimum reissuing fee for copies.
Letters
All letters are $60/page and require 5 business days to complete.
Photocopies
Photocopies up to 5 pages are included in each visit. Any additional photocopies required are invoiced at 10 cents/copy.
Home Delivery
We offer home delivery services from our natural store (http.//naturalrx.ca/). Please ask for current rates. Credit card payment is required for all deliveries. For same day service, orders must be placed and prepared before 12:00 noon.
Supplement Orders
Any requests for Special Orders (Items we do not regularly stock) or requests to hold items for pick up must be prepaid at the time of request. We encourage you to call to request a refill of any of your supplement items prior to running out to allow us time to order your items. Any prescription items required consultation prior to refills please allow 7-14 business days following your visit to receive your prescription.
Note-BHRT hormone refills may take up to 3 weeks from your appointment to your receipt of hormones , thus please schedule your follow up visits at least 3 weeks before you run out of your prescription.
Supplement Return Policy
Please note that we have a thirty-day return policy for all unopened supplements. We cannot return any opened, custom, or specially formulated remedies i.e. custom botanicals (liquids) or home remedies for health regulations purposes. Thank you for understanding.
Please be aware that electronic communication is only for brief clarification questions. New, detailed or urgent concerns that require discussion and case management require an appointment. If there is a need for longer discussion regarding new symptoms or concerns, we will replay by the method that you have contacted us to recommend that you schedule a follow-up appointment, and fees will be applied accordingly at the appointment.
Medical lab testing and records can only be released with your authorization. A Lab or Medical Records Release form is available from Client Services and can be filled out should we need to obtain your previous records. You are responsible for obtaining relevant medical records from other physicians or health-care providers. Please contact them and allow for sufficient time for your request to be completed.
For ease of communication privately, you can direct message your practitioner or administration team as needed in the secure patient portal or call the office for questions regarding your treatment plan. To ensure that your information remains private, Revivelife utilizes a patient portal with your on-line file that contains your health and administration information in a secure HIPAA and PHIPA system. Our goal is to support your success, which relies on open lines of communication. We strive to respond to direct messages through the patient portal within 24-48 business hours. Phone calls are returned by the end of the business day on which your call is received. We do not provide after hours or urgent care. Where required please go to your nearest emergency center.
